How to Make a Referral
- Referrals are accepted on a confidential basis.
- If you suspect possible impairment of a physician, physician assistant, dentist, dental hygienist, podiatrist, or medical psychologist (professional) or have concerns for their safety or the safety of their patients, please contact us.
- HPFL protects the identity of any person who makes a referral on behalf of a professional.
- We receive referrals from multiple sources including hospitals, colleagues, the Louisiana State Board of Medical Examiners (LSBME), Louisiana State Board of Dentistry (LSBD), Louisiana State Board of Veterinary Medicine (LBVM), office staff, treatment centers, and family or friends.
- We also encourage self referrals. If you are in need of assistance, please do not hesitate to call. Any professional who has not been treated or monitored previously can be a participant in the HPFL PHP confidentially, without the knowledge of the LSBME/LSBD/LBVM.
- We accept referrals by phone by calling (888) 743-5747 or via email, fax, or USPS. For referrals made in writing, we request the name and contact information of the individual making the report.
Examples of Common Concerns
Concerns for which receive referrals include, but are not limited to the following:
- Being under the influence of mood altering substances while in the workplace.
- Excessive use of mood altering substances outside the workplace that may affect a professional's ability to practice with skill and safety.
- Mood disorders, such as depression and anxiety or other psychiatric illnesses.
- Workplace disruptive behaviors such as verbal or physical abuse towards staff or patients.
- Boundary violations or sexual inappropriateness with patients (these are mandatory LSBME/LSBD/LBVM reports) .
